If your scheduled hospitalization has been booked, you must undergo a pre-admission procedure.
You have two options for pre-admission:
- You can pre-admit online
For this online readmission, please prepare the following supporting documents, which will be requested during the procedure:
– Photo ID
– Proof of entitlement to social security benefits
– If applicable, a copy of both sides of your health insurance card
Please note that this form is available up to 5 days before your arrival at the hospital (except for teleconsultation and regularization after an emergency visit). After this time, you must go directly to the administration/admissions desk, if you have not received a message authorizing you to go directly to the treatment department.
- You can come to the hospital before the day of your admission to compile your file: whether or not part of the costs will be covered by social security, preparation of medical file….You can also pre-admit after your appointment with the anaesthetist. You will need to go to the admissions department in the yellow sector, door 6.
By preparing your file in advance, you can be directed directly to the department that will take care of you on the day of your appointment. This will also save you time and prevent you from forgetting certain mandatory documents at home, which could jeopardize your hospitalization or consultation.
In the absence of prior admission, you must complete these formalities on the day of your hospitalization or consultation, arriving at least 30 minutes before your appointment time and taking into account possible queuing in the event of heavy traffic.
