Our social assistance service is there to inform, guide and advise you and your family during your hospital stay. Your discharge will be prepared in close collaboration with her, once your doctor has decided on your discharge date. The aim is to help you prepare for your discharge from hospital.

Two types of discharge are possible, either a simple return home or with aids, depending on each individual's situation, or a transfer to a convalescent or rehabilitation center.
It can also intervene in social issues to facilitate access to care and rights related to illness. To do this, it may sometimes need to contact the various government departments involved.
If you need help, don't hesitate to talk to your doctor and the nursing staff, who will be sure to inform the social worker of your situation.
